Information for Employees Affected by the California Wildfires (or Other Natural Disasters)

Direct Deposit
Encourage each employee who does not already have wages directly deposited into their bank account to complete the Direct Deposit Enrollment Authorization Form STD 699, write “Wildfire” or “Natural Disaster” at the top of the form, and submit it to their Human Resources (HR) Office.
To ensure priority processing, the HR Office should email the form to the State Controller's Office (SCO) at PPSDTempOps@sco.ca.gov. Please note: Agencies on ConnectHR, with file upload capability, should follow ConnectHR procedures. Reminder: SCO does not impose restrictions on leave balances or time worked in order to qualify for direct deposit.
Copy of Warrant or Form W-2
As employees relocate, they may need quick access to duplicate warrants and Form W-2s. Please encourage employees to register for Cal Employee Connect (CEC). CEC will give them quick access to view and print warrants and Form W-2s.
Request for Duplicate Controller’s Warrant/Stop Payment
To assist our customers during a declared state of emergency due to the COVID-19 pandemic, wildfires, or other natural disasters, agencies or departments may sign this form on behalf of the employee (payee) for expedited processing of a request for a duplicate warrant (form STD-435 and 113b). Agencies or departments must indicate the nature of the disaster on the top of the STD-435 and 113b (for example, COVID-19, Wildfire, Flood, etc.) and email the form to disbdisasterrelief@sco.ca.gov. Once SCO receives the completed STD-435/113b, it will take 7-14 days to reissue a duplicate warrant. Unmarked forms will be processed as part of our regular workload by the date of the incoming mail.
Note: Any prior forms sent to SCO DISBSTD435Covid19@sco.ca.gov will be accepted. Going forward please mail emergency requests to disbdisasterrelief@sco.ca.gov.
For United States Postal Service Questions:
The State Controller's Office has been made aware that, due to wildfires, some United States Postal Service (USPS) offices in affected areas are closed. Click on the following link to confirm if there is service disruption in your area https://about.usps.com/newsroom/service-alerts/residential/welcome.htm. If you are not able to drive to the alternate post office location listed to retrieve your payment, please contact the agency that initiated your payment for help with submitting a form STD-435 “Request for Duplicate Controller’s Warrant/Stop Payment.”
