REMINDER: Electronic Form W-2 and Form 1095 C Survey
Description: The State Controller's Office, Personnel and Payroll Services Division (PPSD) is always looking for ways to improve the services that we provide. In the future, PPSD would like to offer Form W-2, Wage and Tax statements and Form 1095 C, Employer-Provided Health Insurance Offer and Coverage statements electronically via the Cal Employee Connect (CEC) platform.
Please share this communication with all State Employees and request that they take a few minutes to complete the Electronic Form W-2 and Form 1095 C Survey. Responses to this survey will help PPSD identify how many State Employees are interested in using these services. Please note that responses will not change any of the employee’s current preferences on CEC. Should these services become available, employees will have the opportunity to select their preferences in CEC. This survey will take approximately five (5) minutes. Dear State Employee,
The State Controller's Office, Personnel and Payroll Services Division (PPSD) is always looking for ways to improve the services that they provide. In the future, PPSD would like to offer Form W-2, Wage and Tax statements and Form 1095 C, Employer-Provided Health Insurance Offer and Coverage statements electronically via the Cal Employee Connect (CEC) platform. However, before any efforts are made towards this concept, PPSD would like your opinion concerning its use. Therefore, they have created a survey, which will help identify how many State Employees would be interested in these services. The survey will take approximately five (5) minutes to complete, but will not change your current preferences on CEC. Finally, should these new options become available, you will have the opportunity to select your preferences in the CEC.
