Electronic Form W-2 and Form 1095 C Survey, 

Action Required by Department:    Share the following information with State Employees.

Description: The State Controller's Office, Personnel and Payroll Services Division (PPSD) is always looking for ways to improve the services that we provide. In the future, PPSD would like to offer Form W-2, Wage and Tax statements and Form 1095 C, Employer-Provided Health Insurance Offer and Coverage statements electronically via the Cal Employee Connect (CEC) platform. Please share this communication with all State Employees and request that they take a few minutes to complete the Electronic Form W-2 and Form 1095 C Survey. Responses to this survey will help PPSD identify how many State Employees are interested in using these services. Please note that responses will not change any of the employee’s current preferences on CEC. Should these services become available, employees will have the opportunity to select their preferences in CEC. This survey will take approximately five (5) minutes.

The survey will close on November 30, 2022.
