Revision to Personnel Action Request Form, Standard 680A
Action Required by Department: Share the following information with all departmental human resources (HR) office employees.
Description: The Personnel Action Request (PAR) form, Standard (STD) 680A has been revised to allow for department HR offices to select an A or B option for the Non-Citizen identification in box 545 when completing electronically. The revised STD 680A has a revision date of 01/2021 and is available for use on the Department of General Services website.
