Description: As a reminder, if your HR office has pending Emergency Paid Sick Leave Act (E-PSLA) and Emergency Family Medical Leave Act (E-FMLA) payments, please remember to submit your form Std. 674 along with the calculator saved in PDF format to ppsdepslaefmlaadj@sco.ca.gov. Refer to E-PSLA/E-FMLA Sample 674 documentation for reference.
