Garnishment Forms STD. 639 and STD. 639CFS Completion Guide

Description: The State Controller’s Office (SCO), Personnel and Payroll Services Division (PPSD) created a Garnishment Forms (STD. 639/639CFS) Completion Guide as a supplemental resource to the Payroll Procedures Manual (PPM) Section H 300: Levy on Earnings.

The guide provides information and instructions on the following topics:

• The division's roles within SCO in processing garnishment requests
• Documents to submit based on garnishment type
• How to complete STD. 639 and STD. 639CFS forms
• Required documents when returning warrants to be redeposited for garnishment deductions
• Form completion examples based on common scenarios
