Affordable Care Act – 2021 1095-C Issued Report Update 

Action Required: Share the following information with all human resources (HR) office employees. 

Description: The State Controller’s Office has updated data available in ViewDirect/Mobius View under report ID PDA1782, for reporting year 2021, to reflect the most accurate 1095-C information. Following each annual correction process, the PDA1782 report will reflect the following employee 1095-C records: 

  • Employees with no change to their originally issued Form 1095-C 
  • Employees newly issued a Form 1095-C during the corrections process 
  • Employees issued a corrected Form 1095-C 

Employees whose original Form 1095-C was voided during the corrections process will no longer be reflected on report PDA1782 after the correction process.
